The projected activities to take place in this module include:
Module 1: Tuesday and Thursday
Program Registration and Initial consultation
Development of a “Grow Model” for each participant
Consultation session - “Let’s Talk About Aging in Place”
Expression group intervention - “Give Me Feed Back”
The proposed staff needed for this module includes an occupational therapist and care specialist. The occupational therapist role will be to administer individualized consultation to determine and identify participant’s strengths and weakness in the level of functioning in the home environment. The care specialist will assist the occupational therapist by recording data obtained during individual assessments. The occupational therapist and care specialist will both introduce the components needed to age in place. This particular module will take place in the comfort of the home.
Participants will understand why education, proactive planning and lifestyle changes are needed to age in place.
Getting Back to Me
Intent: Introduce the important components and essential tools needed to holistically age in place safe.
Time Frame: This module will take place during session 1 and 2 of the program
Objective: Participants will be able to identify different lifestyle changes that need to be made at home and in the community and how they affect one’s well-being and level of functioning.
